Description:

Outreach Technology Services previously utilized a printed form for the request of new accounts and computer image/setup. I initiated this projected to implement the use of an online web form to replace the printed form. The project goal was to increase productivity and organization by offering a form that was easier to access, ensures proper information is collected, and allows for more detailed information that was previously limited by the amount of space allotted for then printed form.

My primary Role:

I was the project initiated, manager, and sole technical person on the project. I collaborated with our SDE expert for implementation with the ticketing system, but completed the planning, design, and build of the forms myself.

Project Tasks:

Develop project plan
Design the web forms
Solicit feedback on designs
Develop data flow diagrams
Research php and java script for functional features
Develop/code the web pages
Solicit feedback on page functionality
Revise web pages
Document page functionality and configuration
Collaborate with peers on integration with ticketing system
Develop training and communication documentations
Monitor performance and modify as needed
